This could also be psoriasis, eczema, or sebhorreic dermatitis; it's not necessarily rosacea, but it definitely could be. Flaking is generally more associated with psoriasis and eczema than rosacea. To avoid a potential trip to the dermatologist, you can actually use a dandruff shampoo (containing pyrithione zinc) as a facial wash -- just some regular Head and Shoulders Sensitive Care Shampoo. This is a "secret" common off-label treatment recommended by dermatologists for these conditions when facial skin is very sensitive, dry, and irritated. Do not use a facial scrub of *any* kind. Although this may be tempting as a method of dealing with the flaking, it will only continue aggravate the condition. You can also use this moisturizer: Dermarest Eczema Moisturizer for Sensitive Skin. It's made for eczema but will help symptom relief with the other conditions mentioned. If these don't help to resolve the problem within a week of using them, get to a dermatologist for treatment and exact identification of the problem.

take him to the dermatologist

He should definitely see a dermatologist. There are medicines out there that will really help. My sister has it, and she also just started using "Dr. Bronners" soaps, specifically the peppermint type, and believes that it has also helped clear it up.

He needs to see a dermatologist. I have rosacea, and there is no cure, only treatments to control the inflammation. The redness can be treated with lasers, which are usually not covered by insurance. It may take a few tries at different medications to find the right one for him. If he has it, chances are one of his parents also has it as it runs in families. Left untreated, it can lead to scaring, and can change the shape of his nose, making it bumpy. Hope this helps.
