She probably will .. but it sounds like that doesn't matter to her right now. I'm not sure you can force her into detox, and perhaps she is already under treatment if she is on methadone. I think most of all what she needs is help with her emotional health.. if you can get her to a good psychotherapist and a support group that might help.

The sad thing is that you cannot really save her if she doesn't want to be saved .. but also you can make things worse if you make things too easy for her .. like where does she get the money for her drugs? Is she living at home and supported by you/familly? Don't enable her by paying for stuff that makes it easier for her to get her drugs.

And in addition to checking out help for her, e.g. Narcotics Anonymous, please also look into some guidance for you and the rest of the family as addictions don't just affect the addict, you are all caught up with it and it can destroy you too if you let it. I have a good idea of what a terrible emotional strain you all must be going through. So find out about what support is around for families of addicts, because that will give you strength, ideas, hope and guidance on how to go on, on what to do and what not to do.

I'm so sorry to hear this, it must be so painful for you. I hope you can get some help. I might have some links on my website you can check it out below for starters and see if you find something useful. It's actually a website for survivors of child abuse, but a lot of us survivors have problems with addictions so I have got some information on drugs and alcohol there that you may find useful.

oh yes, very bad news, start on your will at once

Will she die young?!
All signs point to yes.

talk to a doctor

If it's someone you care about you better get her some help.

She definitely could die prematurely if she keeps on. The body requires certain fuels - vitamins, minerals, proteins, fats, etc - to function properly. Without those, the body gets weak, immune system goes down, organs don't work as well.

Not only is your friend not getting the fuels she needs to keep her healthy, but her drug use is causing further damage to an already compromised system.

A friend of mine died after he stopped eating pretty much anything for months, because his addiction was such that he didn't really want anything else. He did finally straighten up, but it was too much and too late for him, and his organs failed. Very sad.

Even if your sister manages physically, drugs wreak havoc on a person's mental faculties, which makes it very hard for a person to function reasonably and rationally.

Your sister needs rehabilitation. Detox and therapy, a regular psychiatrist, a support group, and a family and friends that will be there for her. I hope she can get it.

edit.

As to what to do, I would try to look into finding a psychiatrist or support group or rehabilitation center who can give you advice on proceeding with some concrete plan to get your sister some real help.

And as much as it may seem terrible, jail and/or involuntary commitment can save people's lives when they've gone so far down that nothing else will stop them.
