You may have an allergy to the elastic in your underwear. Years ago I had a similiar problem. Be careful about buying panties. I was able to find those that had the elastic covered by cotton fabric. That would probably help you too.
Get bigger clothes, it sounds like either your underwear or pants may be too tight.
You need to go to the doctor. How big is the hole...depth and diameter? Are you bleeding from this hole? Does it resemble a freshly popped blister? You should not be ashamed about this ailment...doctors will not be shocked or judge you. Don't be afraid to tell your mom either...that is what moms are for.
