It all depends on the thickness of the clothes, the amplification of the stethoscope, and the hearing and ability of the physician. Each layer of cloth between the stethoscope and the heart decreases the sounds transmitted, but it is possible to hear everything well. If you are concerned, ask the doctor, or wear a thin tshirt on your next visit.
