How do I make him take one, and what kind of diseases can he get if he doesn't take one. He hasn't taken one for over a month now.

when i was a kid i maybe went 4 days without one, and my stepmom threw me in with my clothes on in freezing cold water. im not saying to do that, but that worked for me.
maybe one night while he is sleeping, take some food coloring, and drip it all over his face, and then take a sharpie and write all over his arms as much as you can. sharpie will come off, and food dye will come off if you start cleaning it up immediately.

Get some friends and drag him outside with the hose and some detergent. When you're done tell him that's the way its going to be once a week unless he takes one on his own.
I wouldn't let him in your house the smell of body odor is horrific. Lice, and if open sores infection , body lice, just nasty things I don't want to know !!!!!!!
Tell him that if he doesnt take a shower he's gonna get worms, and that is just gross. lol, why wont he take a shower?? If i were you i'd get the hose out and spray him down!!!!
My lil' brother went threw that....We use to strip him to his undies and throw him in the bath tub full of water. He finally started taking them on his own. He got tired of his 3 sisters undressing him and telling his friends he was funky.
tell him that he'll die out of bad odor inhalation from his body odor if he doesn't wash himshelf. haha
Tell him if he won't take a shower he'll get Leprosy, and his skin will start to fall off. Like that one Simpson's episode when Lisa tried to get Homer and Bart to clean the house by making them think they have leprosy. Also you can knock him out with some chloroform and wash him yourself.
Tell him the girls will not want him because he smells (unless he is really young and doesn't want the girls to like him) tell him no one will want to play with him becuase he smells. Don't try to convince him of it or admit it, just tell him and leave him alone to think about it.
If he isn't into the social stuff, tell him that he is getting his bed dirty and that he is sleeping in his own filth.
If that doesn't work, tell him the neighbors are going to report a suspicious smell coming from the house and that the police are going to come and investigate
Take him to a car wash and hose him down.

How old is he? Do yo live somewhee warm - have a football or soccer game, or game of tag in the mud. Then everyone has to shower afterward( to clean the mud out of any cuts or scrapes you might have on your body). In snowy areas, have snow wars till your clothes get damp. Then everybody needs a hot shower to warm up. Did you ever get him to tell you why he won't shower? Is it just a little boy thing or does he really have an issue?
he can get heaps of gross diseases, how old is he? no girls will go for him if he stinks...
Bribe him!!! If not, would he wash up in the sink with a washcloth and soap?
