First, notify the Director of Nursing. Provide documentation ( write down the issues). This should be handled quickly. If not, contact the Administrator. This issue is a violation of patients' rights. If the situation is not resolved, call The State, Board of Health. I hope that this is useful.
i think you will have to contact the state board
report everything and they will go check it out and hopefully get something done about it.
i would talk to the director about your concerns and if they don't do anything i would go to the state board and complain
call the county code enforcement and health dept. good luck.
