Lousy sewage and the presence of mosquitoes. It was thought to be on the way out in the 1950s, but Rachel Carson's fallacious book, Silent Spring, caused DDT [the best mosquito control agent ever] to be banned.
In Africa, governments are now again using DDT, especially in local open sewers and open bodies of water [mosquitoes breed in still watery]. In addition, they are issuing free mosquito nets.
Many attempts have been made at a vaccine, but the infecting agent has many sub types, and also has different lifecycles. A vaccine would need to cover the multiple serotypes and their variant shapes in the different blood life cycles.
There is a man in the greater DC area who may have done just that, but the jury is still out.
The other thing that can be done is to take prophlaxis. Traditionally that has been quinine, but recently there has been a new herbal drug from China that shows great promise.
