I'll let you know this straight up. I am not a doctor but my brother had a pilonidan sinus. His was in the same place as yours. (by what you describe) That is the most common place, infact I have never heard of them being anywhere else.
He had his operated on. You've probably been told of the procedure, but incase not, they basically cut a hole to drain the fluid. My brother had to put gause in it for about 2 weeks i think, and had a bandage over it. The nurse had to come to our home every 2nd day (it probably would have been every day if my mum wasn't a nurse) and she made sure the wound wasn't infected.
It's caused by an ingrown hair, and only happens to men. They used to call it jeeps disease because apparently it's often caused by bouncing up and down on that area, like what would happen if you were 4wheel driving in a jeep.
I don't think there would be any alternative treatment.
I suggest that you get the surgery before it gets any worse.
My brother complained of a sore back for a couple of weeks, then one day he couldn't walk, and that is when he was diagnosed with it.
He hasn't got it back since and the op was 4 years ago now.
Hope this helps.

operation or get it lanced every time it swells up.......
