Don't waste your money on expensive department store cosmetic brands. They are very good at "making good skin look better", but they are really aren't good at treating skin which requires clinical attention. They all have the same active ingredients, anyway: salicylic acid, benzoyl peroxide, or sulfur.

If he hasn't responded to the products you've mentioned, then he really does need to see a doctor/dermatologist. You might consider trying a product called SalAc; you can buy it from Dermstore.com or Amazon.com. Many dermatologists recommend it, and it is good treatment. He can follow it with a 5% benzoyl peroxide treatment.

As far as how to approach him, just buy it, and leave it and bathroom with a note attached, saying, "To make you look sexier!" Subtlety is everything. Don't make a big deal about it. If he has insurance of his own or through his parents, just ask him if his skin bothers him, you'd be happy to go to a dermatologist with him -- that you like him just the way he is but that you understand his acne bothers him. Be encouraging -- but not nagging. Good luck!

He may need something stronger, see a dr!!

He might be deficient in the mineral zinc so a good vitamin and mineral supplement might help. Diet affects skin more than anything so making sure he avoids eating a lot of fast food. He needs to drink water and to eat more fruits and vegetables. I don't know how you can approach him without hurting his feelings.

He needs to see a dermatologist. That's his only best choice.

Listen, go to a dermatologist and ask for medication. I PROMISE YOU this will work. However, mentioning it to him? Just tell him you care that much about him. Although I think he'd have to be the one to ask for the meds.
