What is the correct way to ask for a doctor's note?!


Question: I am currently taking medication that I can't miss a dose on.
I left work this morning because I needed to come home to take it. This medicine is very important because it controls seizures and if I miss a dose I am exposed to them.

I want to call my "busy" neurologist and let him know that I need this note, saying my problem and fax it to my boss.

I just don't know how to go about it. Please help! Thanks!!!

I agree that you should simply tell your neurologist what you told us. This is not a problem and busy or not (ALL neuros are busy) your are his patient after all.

Just ask. Let him know that you would like you workplace to know you are prone to seizures in case you ever accidentally miss a dose or something triggers an episode.

say just what you said on here. You forgot to take your seizure medicine and had to go home to take it and now you need a note to fax to your boss because they require a doctors excuse. They will understand.

Phone the secretary. You're not the first patient who needs a note for his employer, school, government, etc.

However, I have rheumatoid arthritis and every morning, I fill an empty bottle with the pills I need to go through the day, roughly 40 pills or so. I carry the bottle with me everywhere I go.

If you don't have an empty bottle that is small enough, you might ask your pharmacist or other family members.
