Question: OK well im going on a trip and i have no idea what it means by excusion leader ,excursion dates. Like it says date of last tetanus shot: and right beside it it says excursion dates :S so please hlp me my parents dont speak english so i have to fill it our myself.. so please help thank yo so much!:)

It is when you leave the mode of transportation you are taking and go sight seeing, etc. For example if you are on a train or boat, you might leave them to go sight seeing. The excursion leader would be the person taking you on the sight seeing trip.
This means anytime you leave your mode of transportation .
On a boat it would be if you left the boat and went on land to sight see or go shopping.
Just because your parents don't speak English doesn't mean they can't help you fill the form out. Have you thought about translating whatever form it is you're reading to their native tongue? I bet they would love to help you, and know what's going on.
Excursion leader is the person leading the trip. Excursion dates are the various dates of the trips.
Tetanus shot is a shot you probably need since it only lasts 10 years, so you'll need a booster shot. Tetanus is a bacterial infection that can cause lockjaw.
