They either are told by a person that they found out they have it and should go get tested.

They suspect it and go get tested.

Or they exeperience symptoms of it and go get tested.

Either way, you have to be tested to know for sure that it has been contracted.

The only way to know for certain is to get tested. I firmly believe that if anyone has EVER had unprotected sex then they should get tested.
