Ben J - obesity does not directly kill anyone. It's more like a sign that other health problems are present, like diabetes, high blood pressure, high cholesterol etc. It's these conditions which do the killing. Being obese in itself doesn't mean that you're sick, it just means that you're more likely to have or develop serious health problems.
Mental disorder - serious depression. Not many things are worse than feeling completely hopeless, and many people don't understand that you're really sick.
Physical - having read a biography of a haemophiliac who contracted HIV from a blood transfusion, I would have to say AIDS. This book didn't spare any details and really opened my eyes to what people go through.

The #1 worst mental disorder is anorexia... as far as death rates are concerned. And I'm talking about "real" anorexia as clinically defined, not just skinny teenagers who banter around with the term.
I'm not sure about other health problems.

there are lots of lifelong burdens of course, but with this one a person is ostracized from society like a leper as is his/her family.
I would have to say AIDS or HIV. Not only is the the worst health disorder but it is the most feared by humans. This disease is hereditary and is contagious
I would say Alzheimer's. Knowing you're slipping away and can't do anything about it to the point you're trapped in your own body. It's a horrible way to die.
OCD could also be just a horrible. Only if you had a whole bunch of ticks though that made it really hard to function...that's not much of a way to live.

I think cervical cancer for women is one of the worst diseases. My grandmother passed away from it and it just creeps up on you and within a few months, your gone.

or cystic fibrosis because when a baby has it they have to be thumped on the back to remove mucus as the scillia on the cells are not there and so the lungs slowly fill with mucus
down syndrome. mainly because so many people have it but so many are uneducated.
