The doctor will tell you if and how you will need to hold your penis.
I would highly recommend that you ask to see a male doctor. I have been for 2 check ups in the past, 1 male doctor and 1 female doctor. While the female doctor was not necessarily brutal, the male doctor was more careful when examining the family jewels (obvious reasons why).
You don't have to reveal to the receptionist why you want to see the doctor.
If you think that something is not normal down there, go to your doctors, let him examine, then if need be he will refer you to a specialist in hospital.
