1. How long would he survive before getting to the hospital?
2. What would the docs do at the hospital? Surgery perhaps. The question then is what would the surgery do? Plug the hole? Would the heart vessels got affected?
3. What would be the long term effect? A weak heart? A heart transplant eventually?

It really depends on the where the heart was penetrated and what damage was done. If you're talking a 22 caliber without mushrooming lodge into the left ventricular wall without involvement of any arterial pathways, then it is a simple procedure and the patient would probably live. If you're talking high velocity jacketed military rounds like a 7.62 MM from an AK that penetrated several layers of heart tissue along with impelling bone fragments, and then mushrooming and blowing out an exit wound, taking a substantial amount of heart tissue with it, then the patient would most likely die.
