Can someone tell me what an actual order for xrays from a doctor would look like!


Question: Can someone tell me what an actual order for xrays from a doctor would look like or say!.!?
I am in a limited scope xray program where I am trying to put together a project which i would like to include a mock order from the "doctor" requesting xrays!. Can someone please help me!.Www@Answer-Health@Com

Many times, a referring physician will write an order for an imaging study on their prescription pads!. They use the same forms that they give to you when you need medications from a pharmacy!.

The easiest, most basic doctor's order will say "chest x-ray" and "SOB" or "cough"!. The doctor's signature will be at the bottom, and that is about it!

You could also call an imaging facility in your area, and ask if you can look at one of their requisitions!. You can look in your yellow pages, under "x-ray" or "physician - radiologist"!. Tell the person answering the phone (usually the secretary) about your project!.Www@Answer-Health@Com

Just the name of the dr!., area he/she decides is the parts of the body to be x-rayed!. Ex: lumbar would be shown for a portion of the back of a human!. Other parts of the body have specific names also!. Measurements would be given if necessary to describe the part!.Www@Answer-Health@Com

I know the abbrevation for x-rays is XR!. You could also say something like the pt (patient) had pain in his/her left iliac region(which is your left hip area) d/t (due to) a fall he/she had!. ROM (range of motion) is limited, blah blah!. You could go on forever!. Hope this helps someWww@Answer-Health@Com
