I found a good form on Lantus site!. Easy to down load,full page per week, so is easy to put the numbers in without our glasses!.

Can't recommend it as I've never used it, but take at look at the form in this link and see if it works for you!.
Another option would be just to create your own in a Excel Spreadsheet!. Save it to your desktop so you can access it quickly!.
