Your doctor can remove a small tumor to have it biopsied, or if it is larger, he will do a "fine needle aspiration" to pull some cells out. Either way, the cells are fixed to a slide, stained and then viewed on a 10X microscope. Any malignant cells will stain differently than a normal cell and the malignant cells also have a different configuration (they look different). So if your doctor has done a biopsy, he will be able to tell if it is malignant (cancer) or benign (not cancer), not the other way around.
