Does nybody know a site in which u have to fill a questionare about ur health n get help for ur condition?

there is a organization formed by the top surgeons of U S (not sure) which has brought out a form or a questionare.. in this questionare u have to describe ur disease or tell them how u feel and then they look at ur case, and try to help u to bring out a solution to ur case... the name is AIMS but that too i am not sure..
if nybody frm u guys know plz lemme know abt it, its fine even if its not the same insitution but does some work like that
