My uncle who is 77 yr old has had burning urine and a fever for 5days with a clear anal discharge and chills??

While urinating he feels a burning sensation and he says he hears "popping" sounds from his anus every 1-2 seconds with a clear discharge. When he is done urinating the discharge stops. His fever has risen from 98.6 yesterday to 100 today. His doctor has him on Cipro -- he took 1 on Monday, and 2 a day Tues on for 10days. What possible infection or problem could he be having that would contribute to the strange discharge; do UTI's sometimes have this unpleasant side effect? Thank you for your help!!
