As we age, our hearts become more worn - just like any pump!. And so become less efficient at pumping blood round the system!.

An old heart can still pump blood, but less of it, and at a lower pressure!. (Think of a fire-hose, and how high it can direct a stream of water!.!.!. A new one can pump hundreds of gallons per minute to 200 ft high, but an old clapped-out one perhaps only perhaps only a third of that amount, and only 50 ft high!.!.!.)
So while your grandmother's heart can still pump a certain amount of blood round, the amount declines when additional pressure is needed -simply because of internal leaks and seals, and weak muscles etc!.!.!. so the heart falters!.!. it tries, and does its best, but simply isn't capable of too much work, and the pressure drops!.!.

The big danger is that doctors, not understanding the mechanism of how it works, often give beta-blockers to patients like your grandmother in the mistaken belief that lowering pulse rate "reduces the work the heart has to do"!. This is quite wrong, and in many cases kills the patient, because lowering pulse rate reduces cardiac efficiency, it has to work harder, and since it can't, cardiac failure ensues!.
Do ask, if you need more!.Www@Answer-Health@Com
