There's probably a lot because he drinks, smokes, and stepped on a needle and could have gotten something from that!.

I don't know who "he" is, but he's probably lying to you!. He won't tell you what he has that's killing him, but h tells you he stepped on a needle!? I don't believe it for a second!.

If he drinks and smokes, it's probably either liver or lung cancer, but since he told you that it wasn't, we could rule that out!.
Stepped on a needle - hopefully he did not contract an STD that doesn't have a cure because that would be deadly!.
