What do you think of someone getting Weight Loss Surgery with a BMI of 46?

Exercising everyday and eating less and healthier has not worked at all.

That is what we call "morbid obesity"...
You dont specify what type of surgery, that particular person will have.
In the past 20 years or so, sucking fat away from the subcutaneous tissue of the body, brought upon some relief (however temporarily) to the problem, that we have to take seriously....
A person with that body weight and body mass,index, is around 2.2 square meters in body surface...
It would be a great risk for a lenghty liposuction , mainly because the duration of anesthesia.
Endoscopic (not opening anything, and using a tube through the mouth) can place a saline "baloon" inside of the stomach, for instance, decreasing the capacity of the stomach, and giving the sensation of "saciety" or fullness with small amounts of food...
There are other procedures, such as bandaging the stomach through 3 small incisions in the abdomen (laparoscopy), procedure that takes in average 30 to 40 minutes.....

That is NOT a substiture for dieting and mild exercising after surgery, under supervision..... and will need some vitamins such as thiamine, folic acid, and B12, as welll as elements (iron sulphate) after the procedure..whatever that might be...
Its only guess work, since you didnt mention the type of procedure he or she will be undergoing, to comment such, in detail...
