Answers:
May vary by state... (California) but this is one for Biopsy for a single lobe
31628. Bronchoscopy, rigid or flexible, with or without fluoroscopic guidance; with transbronchial lung biopsy(s), single lobe $449.40 $196.62
CPT Assistant.Volume 14, Issue 9, September 2004
CPT Assistant.Volume 15, Issue 8, August 2005
CPT Assistant.Volume 11, Issue 6, June 2001
For each additional lobe:
31632. Bronchoscopy, rigid or flexible, with or without fluoroscopic guidance; with transbronchial lung biopsy(s), each additional lobe (List separately in addition to code for primary procedure) $79.52 $56.26
CPT Assistant.Volume 15, Issue 8, August 2005
HOWEVER, Needle Biopsy is a different code... so if it was done with a Wang Needle instead of forceps/alligator forceps, it's this one:
31633. Bronchoscopy, rigid or flexible, with or without fluoroscopic guidance; with transbronchial needle aspiration biopsy(s), each additional lobe (List separately in addition to code for primary procedure) $93.89 $69.32
CPT Assistant.Volume 14, Issue 5, May 2004
CPT Assistant.Volume 14, Issue 7, July 2004
CPT Assistant.Volume 14, Issue 9, September 2004
CPT Assistant.Volume 15, Issue 8, August 2005

I'll see if I can find the rest...

I think this one is for lobectomy:
32663. Thoracoscopy, surgical; with lobectomy, total or segmental $1426.79 $1426.79
CPT Assistant.Volume 4, Issue -1, 1994

Couldn't find one for chest tube, as it depends on what the chest tube is for. If it's for Pneumothorax, it should be this one:
32002. Thoracentesis with insertion of tube with or without water seal (eg, for pneumothorax) (separate procedure) $217.59 $127.19
CPT Assistant.Volume 13, Issue 11, November 2003
