Question: Do i need to list ALL my allergies on a consent form?
im allergic to quite alot of things, when i go on a school trip and on the consent form they ask you for all alergy information do i need to list everything? i will obviously list any medicine and food related items, but im also allergic to parfum im certain soaps/shampoo. do i need to list these on a form if im only going outside where its highly unlikely i will come in contact with such things?

Yes you do, that's why they give you the forms in the first place. If something happened to you and you didn't list the allergy- they wouldn't know what to do. Say someone was standing beside you with that certain perfume on and you fainted, then that person went with you to hospital, you fall into a coma, nobody would know what is wrong with you or how to help you.
I certainly would. In life one cannot presume certain things will, or not, happen. Also, it legally safeguards both you and the school.
