Morphine will still work. You just keep increasing the dose as tolerance develops.
All good hospice nurses should know this.
Many people wind up on extremely high doses which might kill another person,
but it is safe if an experienced nurse or doctor is adjusting the doses.
You simply dose according to relief of symptoms - level of pain control -
and watch the level of consciousness and the number of respirations per minute.

You can switch to dilaudid or fentanyl if you want to pay more.
They are simply thought to be stronger because a smaller number of mg provides
the same pain relief as a higher mg dose of morphine. You can make them all equal by intelligently adjusting the doses - number of mg per unit of time.
With all opiates the doses will need to be adjusted upward as time goes on.
