Answers:
It is all in how the cell or tumor looks under a microscope. Pre-cancerous cells do not look like a 'normal' cell, but they don't yet look odd enough or misshapen enough to be called cancer.
Despite what is being said here, those people that I know who have had 'pre-cancerous' cells have been taken very seriously. One 15 year old boy I know had a secondary pre-cancerous leukemia called Myelodysplastic Syndrome:

Even though this is not considered 'cancer' they still treat it in the same manner as if it were.
Some pre-cancers are considered likely to turn into cancer and are treated more aggressively.
Think of it this way . .
1. a normal cell.
2. normal cell starts to mutate slightly.
3.normal cell mutates a bit more.
4. irregular looking cell.
5. Cell mutation looks misshapen, irregular and no longer recognizable as anything close to normal.
It's much more complicated than that . . as some cancer cells can also mutate slowly (years) or very, very fast (within days).
