I also have a break out in my back and they are small bumps and some are blackheads.

It's possible your bras are not clean enough. Try warmer water, hand-washing them, etc. Be careful to rinse out all the soap.

Also try a back brush with either an antibacterial bar soap or Neutrogena formulated for acne. Don't scrub hard, but suds up real good.

Make sure your clothes are clean--one wearing only, even if it still looks and smells clean. Launder your sheets and towels in the hottest water they can take, and change them at least once a week.

Limited sun exposure--less than 15 minutes--a couple times a week can help back acne a lot, too.
