There is nothing you can do ,I would advice to call the hospice nurse to administer more narcotics .
according to your description ,he has developed right heart failure and what he is going through now either superior vena cava syndrome or thorbosis where the venous return get blocked by tumor or clots which are very common in diffuse malignancy .
Dexamethason also play role in raising blood pressure .
If you are feeling that something need to be done ,you need to take him to emergency room .

That is a dangerously high blood pressure. He could suffer a stroke or worse if his blood pressure stays that high. You should get him to the nearest ER as soon as possible. Call 911 if you have to.
