wow..that is a rough one. Hopefully he really is always using protection with this man and hopefully he is always using protection with his girflriend as well.
Normally I would say to stay out of it but since this is a fatal disease and you know what is going on it makes things different in this situation.
I wonder if there is a way to kind of hint it to her and allow her to find out for herself somehow or maybe you should give your friend an ultimatum and tell him if he does not tell her you will. It would be better coming from him.

This is a very difficult situation. Using condoms does not protect you 100%, and you didn't even mention if he uses protection with his girlfriend, or only with his friend. Even if he uses condoms 100% of the time (and that means his genitals don't get anywhere NEAR his girlfriend's without a condom!) there's still a risk. She deserves to know about that risk so she can make her own decision.
I feel like you should tell her, but you might lose your friendship with this guy.

Tell his girlfriend what is going on because if you don't, she will continue putting herself at risk without even knowing!

yes!!!! he can still get HIV even if the condom doesn't break and if it does he's screwed! Why would he do that to her? Tell him that HE needs to tell her- and if he doesn't then you will. Thats life threatening and seriously messed up? does she know he's gay?
