okay so I just filled in the online form to become an organ donor in the event of my death. I used this form on the nhs website http://www.organdonation.nhs.uk/ukt/how_…
what i want to know is, is this what will get me a donor card or do i need to fill in a different form?

They have a nasty habit of not sending you one.
Try phoning them and asking for one or try the chemist or doctors surgery oh and a blood transfusion centre, you should get one there.
Just found this for you, if you have a printer you can print your own.
