help with my project on a unhealthy cause diabetes perosn?!

so in my food tec class we are learning about healthy eating. we have to do a little questionare on how a person with a disease caused by unhealthy eating and what impact it has had on their lives. if yoiu can help i just want to know-

- what disease you have
-age and ethnicity
-how does it change your daily life
- what changes have you had to make to insure a better future
