CKMB that high is often due to a heart attack, but as ckm1956 already noted, it can be due to other conditions as well. A much more accurate test is for troponin (Tn). Its elevation is almost always due to a heart problem, usually a heart attack. Was that checked?
Enzyme (CK or Tn) elevation is due to something that happened hours to days earlier. These enzymes are released by injury to muscle cells (usually heart muscle), and remain in the blood that long. A decrease in level is seen when the condition that caused the release improves and cell injury stops.
While it's normally VERYspecific to the heart, there are other things that can elevate it.
The unstable angina is concerning. She could also be having ongoing ischemia of the heart.
