Does my boyfriend have a Pilonidal cyst?!

Question: Does my boyfriend have a Pilonidal cyst?
I'm afraid to ask him because he hasn't said anything.
He spent the night a few weeks ago and after he left I noticed stains on my white sheets... kind of brownish-yellow in color. I thought they might be form my menstrual pad but after continued staining after his visits I know otherwise.
One time after giving him a back massage I tugged his shorts down a little and noticed extra, discolored (reddish? purplish? something like that) skin between the very top of his butt crack, and tugged his shorts back up because I was surprised, and a little grossed out. I didn't really get a good look.
My infatuation with his lovely behind led me to notice that he occasionally has a stain/wet spot where that area of his butt crack is.

Could this be a Pilonidal cyst? I'm not sure if it's supposed to leak a color. Should I approach him about going to a doctor about it? He is long overdue for a yearly physical as far as I know. And its a little gross, and tiring to clean the sheets.
Is it possible that the cyst is making his clothing smell? I'm thinking its just his work out clothing that makes his laundry pile smell.
Thanks for your time!

J C - Your diagnostic suspicion sounds pretty sharp and most likely accurate. The very top of the butt crack is the perfect location of a pilonidal cyst (PC). A PC is NOT a sexually transmitted infection and therefore should not cause any embarrassment for anyone - not for him or for you. A PC is considered congenital and due to some inborn hair follicles deep in the skin of that area. The sebaceous skin glands produce the smelly discharge which can easily be infected and sometimes blood-spotted. It NEVER cures itself, and will recover and get worse in the future if untreated. Your boyfriend needs to see his physician and plan for an effective treatment now to produce a permanent cure. Initially drainage now, and possible local excision are the usual means to a cure. You will do him a big favor by telling him you care and encouraging him to get cured. For right now, it would help him to sit in a hot tub of water 30 minutes a day with enough water to cover the top of his butt crack. This keep the wound open and draining, which is important. He could buy some 4x4 inch gauze pads in the pharmacy and keep a couple of such sterile pads in the crack over the opening, changing pads after his Sitz bath.

It either sounds like a cyst or anal leakage. Instead of going the snooping route, I would just ask him. I'm sure it's a delicate issue for him, so approach it with caution and try to be understanding. Refrain from getting angry at any point or that will just put him on the defensive. If he's very adamant about not talking, don't push too hard. If this is something you can get over just give him time, I'm sure he'll open up in conversation before he takes off his pants.
