Question: Have you ever had a bottle of extra potent insulin where you keep getting lower than expected glucose readings?

That sounds suspiciously unlikely. I don't think insulin is a parts-per-million liquid measurement... it's either insulin or it's not insulin. I think the more likely scenario is that your bg meter is out of whack. Have you checked your meter lately? Did you recently open a new bottle of strips and forget to code your meter? And I guess more importantly, if you're sure the math is right and you can be relatively certain what your bg is SUPPOSED to be, but it's lower... do you feel low? Are you shakey, dizzy, or sweaty? I'm sure you know what you feel like when you're low.

And also, have you started working out lately? Working out can lower your insulin resistance meaning you just need less insulin.

If I were you, I would test your meter with that little bottle of glucose solution to make sure your strips are fresh and your meter is reading right. Then test again. If possible, do you have another meter lying around you can check with? (I've accumulated like a half-dozen of them.)

Last possible explanation: are your strips expired? Expired testing strips can make your readings waaaay off... way too high or way too low.
