i think you did good, only, i think its better to have a bigger breakfast and the meals diminish as the day progresses. Go on calorieking.com and add up the things you ate based on portion size and other factors that contribute to the types of calories and number of calories you consumed... you did not do bad at all!! just buy multi grain bread so that you pb&j can be the healthiest it can be next time!!!

You need more calories and for a vegetarian you sure aren't eating many vegetables. :)

The things you eat are healthy, but you need more healthy grains, something that resembles some fat, and extra protein.

you consumed about 1000 calories depending on the bread and milk type. grapes lettuce watermelon and other water filled fruits and vegs have VERY "low cal" under 70 calories per serving.

You didn't consume enough calories, or protein.

triple your protein intake, if not x4

the following are in no way "meals"
water and grapes
salad

Do you have specific goals? Such as weight loss? In my opinion, not too bad, but its very possible you actually aren't eating enough calories. Try to add more rich protein sources such as mixed nuts, low fat tofu products, and boca products since you're a vegetarian. For example, on that salad throw on some walnuts and top it off with an organic oil/vinegar dressing. It'd be very hard to determine the amount of calories you ate since we don't have precise serving sizes, but use any calorie counting site and you should have your answer.
