I have 15 weeks to get in shape and lose 10 punds..what are some good ways to get results?

I need like a workout schedule, and a food schedule that will be easy and effective for a person who sits at a desk 40 hrs. a week and gets out and can only do anything before work and after..I work 8-5 mon-fri.
I really need to tone, and possible lose 10 pounds.

any suggestion will be GREATLY appreciated.
