One important thing to do at the grocery store, especially when you are just starting to make changes, is to read the list of ingredients on the back carefully, and also to read the nutrition information for sugars and salts in addition to calories and fat grams. For instance, many "wheat" breads and bread products are not whole wheat... and you can tell this by the fact that "whole wheat" is not the first (or second) listed ingredient. So buying whole wheat bread is good, but make sure it is real whole wheat.
In addition, foods like yogurt, which are healthy, are often packed with sugar - even the lowfat or nonfat kinds. If you buy yogurt, look for the kind with 10g of sugar or less. This is also true for things like spaghetti sauce and protein bars - they look healthy, but can be loaded with sugar and sodium.
In general, stick to fresh and unprocessed foods as much as possible - like oatmeal (again, check for sugars), fruits and veggies. Drink lots of water, limit (or eliminate) soda and also... watch the frozen foods section - Lean Cuisine and Kashi are good brands to buy - but again, watch the sodium. I think the daily recommendation is 1500mg or less for most people.
