Answers:

Longer isn't better. The quality of your routine is more important.

I hardly ever go over forty minutes with my lifting. I do, however, do thirty mins to an hour of cardio as well... but that's mainly because I'm working up my stamina as much as I can.

Three times a week is sufficient if you're doing a full body workout. Six times a week is good if you're used to it and are doing a more advanced uppper/lower body split. Furthermore, despite what some say, machines are an excellent complement to free weights. I do both. If you want to bulk up, free weights are better, however.

If you really want to know what the most important piece of gym equipment you can get is, it's dumbbells! You can do a full body workout with a good set of dumbbells.
