How do you tell someone you love that they need to get their teeth fixed?

As long as their teeth are healthy, even if they're a little unsightly, it shouldn't matter to someone who loves them. :)
Take me for example, my teeth are spaced out in a really ugly way. They look bad and I know that. However, it actually makes them easier to clean, and therefore very healthy. And braces are around $7,000 at the very least, so it looks like nothing in my mouth is getting fixed anytime soon.
If you're talking about someone who has poor dental hygeine and needs to see a dentist, casually bring up something about needing to go to a trip to the dentist and say something like "I love that fresh feeling after going, don't you?" and if they say no, explain to them something along the lines of "yeah, but I still need to go, don't want my teeth to rot or fall out. :P"
Be gentle, because teeth aren't a very easily "changable" thing. Changing requires time, and a LOT of money if you're talking about things like caps or braces..
