If you have masses of the bacteria all over your teeth in the form of hardened, crusty tartar, there is no way you can clean that off yourself. You need either a dentist or a hygienist to scrape that crud off for you.

After the hard tartar is scraped off, it is up to you to maintain good oral hygiene to prevent the tartar from coming back. Crusty tartar starts out as a white, sticky soft bacterial plaque, and it takes about 12 hours to turn crusty. So the trick is to brush the soft plaque off your teeth before it has a chance to turn crusty. That is why you need to brush your teeth, tongue and gums thorougly before bed, and after each meal.

If your gums are already badly damaged by the bacteria, with deep pocketing that traps plaque, then you might need periodontal surgery.
