The most common method is tray bleaching. Models are taken of your teeth and custom soft trays are made on these models. Bleaching materials are put in these trays and the trays inserted--typically (but not always) before sleep. Any excess bleach squeezed out should be wiped off, as it may be irritating to the gums. This is done nightly for 1-2 weeks.
There are faster (and more expensive) methods, using laser or plasma light activation. These methods (Britesmile and Zoom) use similar bleaching material, but because of light activation they can lighten teeth in a single sitting.
Besides gum irritation, these bleaches also can make the teeth sensitive to hot and (esp.) cold. Because they are done quickly, this is more likely with the light-activated methods. This sensitivity usually resolves however within a couple of weeks.
