If the tooth itself is loose then you have probably lost bone around the root of the tooth (periodontal disease). You need to see a dentist/hygienist for a diagnosis & treatment as other teeth might be headed the same way.
If you have sub-gingival calculus deposits (tartar under the gum or on the root surface) then there is little you can do on your own.
