Answers:

A "hot tooth" is a tooth that has very great internal pressure due to blood engorgement, resulting from severe inflammation of the pulp inside the tooth. Such a hyperemic tooth can be difficult to numb, yes.

Since you mentioned "hot tooth," I am assuming you needed to have a root canal done. If your dentist has trouble numbing the hot tooth, he might do one of two things: Give you an antibiotic prescription to bring the infection in the pulp under control, or refer you to an endodontic specialist who might be better equipped / more experienced on doing root canals on a hot tooth.

If this is a lower-jaw tooth, they are more difficult to numb than upper-jaw teeth by default. The sad truth is some dentists are just no good at doing lower-jaw nerve block injections.

Here's a tip: If your dentist is going to work on your lower-jaw molars, don't let him start working unless half of your lower lip is numb. The lower-jaw molars share the same nerve as half of the lower lip, and if half of your lip hasn't gone numb from the injection, the molars sure as heck aren't numb either.

Good luck.
