My five year old son gets a brown to black staining on his teeth. Dentist has no idea what it is. Do you?

We brush his teeth regular and he rarely drinks anything except water or milk. We have public water. He rarely eats candy.

The first year of his life he was on antibiotics almost constantly due to chronic ear infections

Black and brown staining on teeth is USUALLY caused from something they are eating or drinking and the most common causes are: dark soda like colas, iced tea or hot tea. Being that he is only 5 does he drink any dark koolaids (grape?) some of these can also cause dark staining.
Make a mental note of what he is drinking and make sure he brushes 2-3 times per day. If he is only 5 and already getting these stains he may need your help to brush better so these stained areas don't start turning into cavities at an early age.
Recommendations:
Make sure your dentist gives him a fluoride treatment 1x per year
As soon as his permanent molars erupt make sure your dentist puts sealants on them
If your son ends up being cavity-prone at an early age making sure these things are done will help him retain his permanent teeth longer with less problems.
