I need tooth (No. 6 Right Molar) extraction but LA won't work.?!


Question: I was prescribed METRONIDAZOLE. even after two LA shots, the area was sensitive near the base of the tooth. It is already filled and needs tooth removal. I also have a no. 8 impacted (at 60 degrees approx) molar on same side and is quite painful as well. Dentist told me to remove infected 6 to make space for 8 solving both issues.

It is true that an acute infection will make numbing more difficult, but I usually try more than 2 injections before giving up. The strongest generally available anesthetic is Septocaine (articaine) which often succeeds when other anesthetics fail. I'm not suggesting you tell your dentist what to use, but...
Metronidazole is probably more active against gum infections than it is with infections originating in the tooth. For those I still use a penicillin, or clindamycin for patients who can't take penicillin.
One other thing concerns me--extracting the first molar in the hope that your second molar will move forward is not usually a good idea. The second molar will likely tip forward, not move horizontally, and while this MIGHT (don't bet the farm) allow space for the wisdom tooth to erupt, it will more likely just mess up the bite of the second molar.
Of course you may not be inclined to go to an orthodontist, but there are enough questions here in my mind that I think you should go to another dentist for a second opinion.

Seems like your remaining choices are novacaine, nitrous oxide, or pain. Surely the dentist has other means to deaden the area sufficiently to allow the work to proceed.

Go to an oral surgeon and get put out. It will probably be better, especially since you have an impacted tooth. Why metronidazole?

METRONIDAZOLE IS A VERY STRONG ANTIBIOTIC FOR SERIOUS INFECTIONS.
AFTER A FEW DAYS THE LOCAL ANESTHETIC WILL WORK SO THAT THE TOOTH MAY BE SAFELY EXTRACTED.

Sometimes when an infection is present, the infection creates an acidic environment and when the anesthetic is injected, the basic component of the drug is neutralized/used up before it can effectively pass through the tissues to block the nerve impulses (ie: pain). The Metronidazole is to clear up the infection. Also, if you intend to 'make room' for that 3rd molar, it would probably be best to get a little orthodontic treatment to slide the 2nd molar into the space left by the removed tooth and help the 3rd molar com into the 2nd molar's space so it can come in straight and functional. Then voila, it's like you never lost the 1st molar! Good luck.
