~I would say that the largest effect that water temperature will have in brushing is warm water will help to soften the bristles, for a more gentle toothbrushing experience.

No. There is so little water in the brush that the temp has no effect. The real cleaning is done by the brushing action. You should be flossing too.
I always use cool water..it feels better and more refreshing I think. but I know some people brush in the shower using warm water...I think I heard something about your teeth being whiter with warm water...not sure.
Nope. Technically you don't even need toothpaste! It's merely the friction of the toothbrush moving that removes the bacterial plaque from your teeth.

I don't think it matters, But I use hot and cold water. I think the toothpaste matters not the water.
