Answers:
I don't think you would have much luck talking an oral surgeon into doing much work on your mouth while you are pregnant, little alone doing it with a local anesthesia. If something absolutely, positively must be done, they will usually do it- but only with a local anesthetic. The risks have to outweigh the benefits, and the risk to your unborn child would weigh pretty heavily on the side against it. Even c-sections are done using a spinal, just to avoid the complications of anesthesia. If they won't use a general anesthetic for delivery, where the exposure would be limited, they are not likely to be favorable to do it for something like a tooth extraction. If the tooth needs to be removed to eliminate an infection, then they will do it, as the infection poses a greater risk than the local anesthetic would. Otherwise, most dentists would recommend you postpone dental work until after delivery. Most dentists are not prepared to deal with monitoring a fetus, nor with any of the complications that could arise. While they are experts with teeth, they are not experts with labor, delivery, nor neonate care- nor do they wish to be. So if you need this sort of work done, go with the recommendation of your dentist, along with your obstetrician.
