Answers:

If the abscess is painful you will need antibiotics first to control the infection and the pain. That is called a hot tooth, sometimes a DDS can give anesthesia directly into the chamber to calm it down, but that is also painful. A patient can be worked on with an abscess if no pain is present and that I would say is 90% of the time. Someone who waits until it is so painful, probably procrasctinated or does not see a DDS regularly. Health insurance will not pay, only dental insurance, unless you have a program through your job like flex pay that assists in medical bills. After every root canal the tooth needs a crown, having it pulled is cheaper, but in the long run years from now will cause you problems, fix the tooth right and make payments.
