To answer your question as best I can, I managed a Tattoo and Piercing Clinic. A tongue ring/barbell really has no good points in my opinion. Not only does bacteria collect on it, around it and in the piercing site, it will wear down your teeth and possibly crack the enamel on them. It will make your breath bad and you could also get a stress bump on your tongue that will not go away. There are many more issues as well. To many to list. As a nurse, I have seen many tongue piercings go bad. They are slow to heal. A swollen tongue is a life threatening condition. The effect of a ring or ball hitting against the teeth can cause problems.

If you have good oral hygiene, they are totally sanitary. When you first get one, you are supposed to rinse with Listerine after each time you ear and drink until it is healed. After that, if you rinse with mouthwash and brush regularly, it should be fine. Also take it out and clean it properly. I have never had any issues with mine.
